Cardiac/Pulmonary Health
and Fitness

Flagstaff Medical Center’s Cardiac/Pulmonary Health & Fitness program provides a wide range of comprehensive services to cardiac and pulmonary patients, post-heart attack, coronary artery and bypass patients. Designed to support the physical, emotional and social needs of the patient and family, the program offers a continuum of care to patients in Northern Arizona. Individuals can learn to reduce their risk factors for heart disease and have a specialized plan of care developed by our team.

FMC offers four levels of cardiac and pulmonary fitness programs. A physician is on site at the medical center during exercise sessions. The programs adhere to guidelines of the American Association of Cardiovascular and Pulmonary Rehabilitation.

Professional Care

  • Specialty-trained registered nurses

  • Exercise physiologist

  • Certified nursing assistant

Phase I Cardiac/Program

  • For inpatient cardiac or pulmonary patients

  • Provides comprehensive education regarding patient’s disease, medications, procedures and care issues

  • Provides physical activity as prescribed by doctor

Phase II Cardiac Program

  • This program is designed for cardiac patients who have had a heart attack or a coronary bypass within the last year or have stable angina. Also, new indications for cardiac rehab include valve surgery or stent placement within the last year.

  • Program requires stress test by referring physician or from facility of their choice.

  • Initial evaluation is performed by registered nurse and exercise physiologist.

  • There are up to 36 sessions available of 50 minutes each. The patient is monitored for the entire session; vital signs, heart rhythm and oxygen saturations are monitored.

  • Education is individualized according to the patient’s needs, including: relaxation and stress management techniques, nutrition, medication instruction by a multidisciplinary team. An individualized exercise program is planned.

  • Pulmonary rehabilitation is offered as a monitored Phase II exercise program for those patients with emphysema and chronic bronchitis. Respiratory therapists and RNs provide one-on-one education on home equipment, oxygen use and proper breathing techniques. The goal of pulmonary rehabilitation is to assist the patient in coping with their disease and improving the quality of life.

Phase II Pulmonary Program

  • This program is designed for pulmonary patients with emphysema, chronic bronchitis, asthma with chronic obstructive cases, etc.

  • The initial evaluation includes a 6-minute walking test.

  • Individualized plan of care.

  • Each session is 50 minutes of monitored exercise and includes staff instruction on home equipment, oxygen use, medications and activity.

  • Patients may be instructed in pursed lip breathing, diaphragmatic breathing techniques, chest percussion and postural drainage.

Phase III Cardiac/Pulmonary Program

  • Phase III is for patients who have completed Phase II and want to continue exercise in a group setting.

  • This maintenance program is for the more independent patient.

  • A cardiac monitor is used as needed.

  • Vital signs and pulse oximetry (measures oxygen in the blood) are checked.

  • Usually not covered by insurance (patients are self-pay)

Phase IV Cardiac/Pulmonary Program

  • Phase IV is for patients who have been in Phase III without complications or hospitalization for three months.

  • This maintenance program is for the more independent patient.

  • Vital signs and pulse oximetry (measures oxygen in the blood) are checked.

  • This phase is not covered by insurance (patients are self-pay).

Education
Once a month, patients meet to discuss the challenges of dealing with chronic cardiac and pulmonary diseases. This informal setting provides peer socialization as well as a support network for participants and family members.
